We partner with you to find the best care options and offer additional services, as needed, so you don't have to waste time and money figuring out the next steps. Our goal is to offer compassionate care, expertise and guidance for your family during this process.  

We work on a referral basis with care providers and facilities, allowing us to offer our matching assessments and advisory services free of charge to our clients. For additional services you may choose to add such as moving, legal, technology and more - we have partnered with specialists who are available to you for a fee, depending on the request. The only thing Care Match charges for directly is if you want to add on ongoing support and care management for a flat hourly rate.  

 We provide recommendations for various care options, including:

- Skilled nursing facilities

- In-home case management 

We thoroughly vet all caregivers and facilities we recommend. This includes checking qualifications, certifications, and references, as well as conducting thorough tours and interviews to ensure they meet our high standards of care.  

Our process begins with a comprehensive in-person assessment of the senior's needs, preferences and circumstances. Based on this information, we provide personalized recommendations for senior living facilities. We schedule and guide you personally on tours of each recommended facility, walk you through the process and assist with your transition.  

Absolutely! We provide ongoing support throughout the transition process, including assistance with paperwork, understanding financial considerations, and ensuring communication between families, caregivers and healthcare providers.
